Contract Form definition

Contract Form means the form signed by the Communications Provider and BT to enter into this Contract;
Contract Form means the prototype of a "contract" and any associated riders and endorsements filed with the commissioner by a health care service contractor or health maintenance organization.

More Definitions of Contract Form

Contract Form means the form setting out the principal terms of a Contract (save for a purchase order), as agreed between the Company and the Supplier in accordance with these Conditions.
