Evaluation Forms a. Performance evaluation forms will at a minimum include the following:
Evaluation Forms. All formal evaluations shall be filed in the employee’s official personnel file and are available for the employee’s inspection by appointment in accordance with Article 15 – Personnel Files. A copy of the completed evaluation form shall be given to the employee. Effective 1/1/2010, all evaluation scales will be changed from the three ratings of Unsatisfactory, Satisfactory, and Outstanding to five ratings of Unsatisfactory, Needs Improvement, Meets Expectations, Exceeds Expectations, and Outstanding. The evaluation form shall be as shown in Appendix D attached and as of 1/1/2010 shall be available electronically.
Evaluation Forms. 8.2.1 Forms used for evaluation shall be prepared by the District and shall be considered a part of this Agreement (see Exhibit A).
Evaluation Forms. Forms which are used by the College for recording and conducting employee evaluations shall be furnished to the Union.
Evaluation Forms. 8.3.5.1 Classified employees shall be evaluated using the appropriate Classified Employee Performance Evaluation forms.
Evaluation Forms. All evaluations conducted pursuant to this Article IV shall be conducted with the use of such forms as appear at Appendices C and D of this Agreement. Prior to an evaluation becoming a part of an administrator’s permanent personnel record, he or she must be given an opportunity to review and respond to any recommendations or comments that are recorded on the evaluation form.
Evaluation Forms. Evaluation forms shall be reviewed by CSEA to ensure uniformity and fairness to all classified employees. Changes in the evaluation form shall be agreed to by both parties to this Agreement. (see Appendix E)
Evaluation Forms. After the effective date of this Agreement, the Labor Management Committee will work together to annually review evaluation forms for each classification in the bargaining unit.
Evaluation Forms. The evaluation forms referred to in this Article are attached as Appendix F in the master agreement.
Evaluation Forms. 8.3.5.1 The completed evaluation will be retained in the employee’s personnel file.
