DCHD specifically reserves the right to withhold the certificate of recognition until the Resident is in compliance with all rules, regulations and personnel policies, including but not limited to, the payment of any and all money due DCHD, return of pager, ID tags/badges, books, equipment, parking tags/stickers and supplies/equipment, and completion of all In Basket items and medical records in accordance with the rules and regulations of DCHD and Medical Staff.
Vacation requests for Family Nurse Practitioners in the Hospital and Clinics Division which are received in the Medical Staff office thirty-five (35) days in advance will be responded to within ten (10) days of the receipt of the request and will be approved or denied based on overall FNP staffing considerations for the time requested.
This Agreement applies to all members of EvergreenHealth’s workforce, including all employees, members of the Medical Staff, temporary and contract employees, volunteers and students.
The Resident acknowledges that during the term of this Agreement, he/she shall satisfy all requirements imposed by PCH and/or PCH’s Medical Staff as required for participation in the Program.
