Eligible Patients definition

Eligible Patients means the estimated maximum number of potential Care Plus patients for which a PHO received Care Plus funding according to the Care Plus funding formula.
Eligible Patients means those patients designated as eligible recipients in the gift instrument or other documentation establishing the Fund or, to the extent such designation is not made or is no longer applicable for any reason, patients who demonstrate financial need as determined by the Hospital following examination of available income, assets and such other information as may be required.

  • Customer and the Practitioners shall retain the ultimate authority and responsibility for the provision of all professional medical services provided to Eligible Patients to the fullest extent required by any applicable law, standards of professional practice or licensing authority.

  • Customer shall use commercially reasonable efforts to persuade Eligible Patients to consent to receive CCM Services and to actually accept CCM Services each month from a Case Manager.

  • Covered Entity will inform Eligible Patients that they are free to choose a pharmacy provider of their choice and, at its discretion, advise Eligible Patients that they may be eligible for a discount on certain prescription drugs at Covered Entity’s authorized 340B pharmacy locations.

  • Covered Entity’s compliance with laws shall include establishing appropriate control procedures to ensure that only Eligible Patients receive 340B Drugs from Covered Entity’s authorized 340B pharmacy locations.

  • For those Eligible Patients whose prescriptions are reimbursable by a Private Insurer, Walgreens is responsible to process and bill such Private Insurer at the existing Contracted Rates.

  • Client hereby grants to Company and SMG a nonexclusive, perpetual, royalty-free, worldwide right and license to use the data collected in the Surveys for any purpose whatsoever and in any form or medium, provided, however, that such data does not compromise the confidentiality of Client or the Eligible Patients who participated in the Surveys.

  • Covered Entity prescribers will provide all Eligible Patients with a valid prescription as required by law which will contain, but not necessarily be limited to, the applicable Covered Entity Location name, address and identification number, the eligible prescriber’s name, and the Eligible Patient’s full name.

  • Company shall make two versions of the Survey available for completion by Eligible Patients.
