STUDENT HEALTH RECORDS. The Board shall provide employees with a list of students in their building(s) with known special medical problems by October 15. The Board’s student medical records along with emergency medical procedures, if any, shall be provided for affected employees on a need-to-know basis, and employees shall hold all information in a student’s medical records in confidence.

STUDENT HEALTH RECORDS. Physicians' written orders and the written authorization of parents or guardians should be filed with the student's cumulative health record. An appropriate summary completed at least once every school year for each medication prescribed and taken should become part of the student's health record. Student health records shall be retained in accordance with Board policy {**}EHB and the District’s Data/Records Retention Schedule, {**}EHB-R, or any superseding statutes or regulations. STUDENT HEALTH RECORDS. Any student participating in a field work experience shall, at the request of Mental Health Facility provide a current statement from his or her physician that the student is in good health and capable of participating in the field work experience. Mental Health Facility, upon request, may require that any student returning from an extended absence caused by illness or injury submit to a physical examination or present a statement from a physician indicating that the student is capable of resuming Mental Health Worker activities. Any such physical examination shall be the financial responsibility of the student. If required by Mental Health Facility, a student participating in a field work experience shall provide verification of annual T. B. screening, immune status for rubeola, rubella, chicken pox, and Hepatitis B (or signed waiver for Hepatitis B).
STUDENT HEALTH RECORDS. Each Party shall maintain the confidentiality of Students’ health information in their possession to the extent required by applicable law and regulation. All charts, data, reports, and records pertaining to the Students created by Hospital in connection with Services under this Agreement shall be and remain the property of Hospital. The obligations of the parties in this Section 3(i) shall survive any termination or expiration of this Agreement.

  • Student Records The School shall maintain student records for current and former students in accordance with the requirements of State and federal law, including the Family Education Rights and Privacy Act, 20 U.S.C. § 1232g, as may be amended from time to time.

  • Client Records 26.2.1 CONTRACTOR shall prepare and maintain accurate and complete records of clients served and dates and type of services provided under the terms of this Contract in a form acceptable to ADMINISTRATOR.

  • Project Records ‌ As further described below, program records include but are not limited to Grantee, financial, and participant records. All project records must be retained for a period of three (3) years after final payment under this grant. All project records are subject to audit pursuant to Section N (4) of this grant agreement. Upon completion of the third year of record retention, the Grantee shall submit all program records to CARB. Hardcopy or electronic records are suitable. Acceptable forms of electronic media include hard drives, CDs, and DVDs. Other forms of electronic media may be allowed based on prior written concurrence from CARB.

  • Education Records Educational Records are official records, files and data directly related to a student and maintained by the school or local education agency, including but not limited to, records encompassing all the material kept in the student’s cumulative folder, such as general identifying data, records of attendance and of academic work completed, records of achievement, and results of evaluative tests, health data, disciplinary status, test protocols and individualized education programs. For purposes of this DPA, Education Records are referred to as Student Data. Personally Identifiable Information (PII): The terms “Personally Identifiable Information” or “PII” has the same meaning as that found in U.C.A § 53E-9-301, and includes both direct identifiers (such as a student’s or other family member’s name, address, student number, or biometric number) and indirect identifiers (such as a student’s date of birth, place of birth, or mother’s maiden name). Indirect identifiers that constitute PII also include metadata or other information that, alone or in combination, is linked or linkable to a specific student that would allow a reasonable person in the school community, who does not have personal knowledge of the relevant circumstances, to identify the student with reasonable certainty. For purposes of this DPA, Personally Identifiable Information shall include the categories of information listed in the definition of Student Data.

  • Personnel Records (A) There shall be only one official personnel file for each employee, which shall be maintained by the employing agency. Information in an employee’s official personnel file may be maintained in electronic as well as paper form.

  • Property Records Subrecipient shall maintain real property inventory records, which clearly identify properties purchased, improved, or sold. Properties retained shall continue to meet eligibility criteria, rental limitations, health, safety and building codes, etc., and shall conform to federal and State regulations.

  • Medical Records Medical records relating to Trial Subjects that are not submitted to Sponsor may include some of the same information as is included in Trial Data; however, Sponsor makes no claim of ownership to those documents or the information they contain. c.
