Primary Care Provider. Pharmacy must notify the patient’s primary care provider, as provided by the individual or the individual’s agent, within fourteen (14) days of the administration of the vaccination. This notification must include the name and address of individual, age if under fourteen (14) years old, name of primary care physician if provided, date vaccine given, amount administered, manufacturer, lot number, expiration date, site of vaccination and route of administration, and the name, address and title of the person administering vaccine.13

Primary Care Provider. 11 Unless prohibited by the individual (or if the individual is a minor, by the individual’s parent or guardian), Pharmacy must provide documentation on the administration of vaccines to the individual’s primary care provider as indicated on the informed consent form.

Prior to executing the Agreement, Provider, Subcontractor and Health Plan shall specify the number of linkages Health Plan may link to Provider.
