Pay Slip. A pay slip shall be supplied whenever the take-home pay of the employee changes or at the request of the individual employee.

Remuneration payments shall be accompanied by a pay slip providing the following information: The Second Unit Director’s social insurance number; The Second Unit Director’s name and address; The production company’s name and address, telephone and fax numbers; The title of the production; The function held; The time worked; The Gross Remuneration; The deductions; The net remuneration; The fringe benefits.

Pay Slip. The rate or rates of pay, hours of work, details for overtime hours and all necessary and pertinent information will be furnished to each employee on their pay statement so that the employee can clearly understand how their total pay was calculated. The employees' acceptance of a pay slip does not waive payment of all or part of the wages owed them.
Pay Slip. The company will provide each employee with a pay slip on pay day showing: • the total amount of wages due; • the nature and amount of any deductions made; and • the net amount being paid.

The pay slip shall list all components of the employee's salary as required by the Wage Protection Law, 5718-1958, including the deductions made from the employee's salary.

Where it is possible with the accounting equipment now in use, the Company agrees to show the number of miles and the amount earned in miles, the number of regular hours and the amount earned and the number of overtime hours and the amount earned. If the Company installs new equipment, it is agreed it will install equipment that will supply the above-mentioned information. If the Company is presently supplying additional information over that listed above, it will continue to do so. ARTICLE TIME CLOCKS If the Company employs five (5) or more employees at any one (1) terminal a time clock shall be provided and shall be accessible to employees. All employees must use it. There shall be a book-in, book-out sheet which all drivers must sign when they are arriving or leaving the terminal which will read terminal, truck number, point of origin or destination and punch mark the time clock, when available.
